Scope of this policy
This policy applies to visitors of codentramcb.com and to people who contact us about our billing and coding services. It does not describe how we handle protected health information on behalf of healthcare practices. That information is governed by our agreements with those practices and by applicable law, as explained in Section 3.
Information we collect
Information you give us. When you call, write to or otherwise contact us, we may collect your name, practice name, role, phone number, mailing or email address and any details you choose to share about your practice and billing needs.
Information collected automatically. Like most websites, our hosting provider records technical data when a page is requested, such as IP address, browser type, device type, operating system, referring page, pages viewed and the date and time of the visit. We use this for security, troubleshooting and understanding how the site is used.
What we do not collect here. This website does not have patient intake forms, and we do not intend to collect health information about you through it.
Protected health information
In delivering billing and coding services, Codentra may act as a business associate of a healthcare provider. When we do, protected health information (PHI) is handled under a written business associate agreement with that provider and in line with the Health Insurance Portability and Accountability Act (HIPAA) and related regulations. That handling is separate from this policy.
Please do not send patient names, diagnoses, insurance identifiers or other PHI to us through this website, by ordinary email or by text message. During onboarding we will give you a secure method for sharing the information needed to start work.
How we use information
We use the information described above to:
- respond to inquiries and schedule introductory calls;
- prepare proposals and deliver the services you request;
- operate, secure and improve our website and communications;
- detect and prevent fraud, abuse and security incidents;
- meet legal, accounting and regulatory obligations.
Cookies and third-party services
This website does not set advertising or cross-site tracking cookies of its own. If we add analytics or similar tools in the future, we will update this policy and, where required, ask for your consent.
The site loads typefaces from Google Fonts. When your browser requests them, Google receives your IP address and basic browser information, and Google's own privacy policy applies to that exchange. Our hosting provider also processes technical data to serve the site.
How we share information
We do not sell personal information. We may share it with service providers who help us run our business, such as hosting, telephone and communications vendors, under obligations to protect it. We may also disclose information when required by law, to respond to lawful requests, to protect rights, property and safety, or in connection with a merger, financing or sale of our business.
Data retention
We keep personal information only as long as needed for the purposes in this policy, including to meet legal, tax and contractual requirements, and then delete or de-identify it. Records created in the course of client services are retained according to the applicable client agreement and law.
Security
We use administrative, technical and physical safeguards designed to protect information against loss, misuse and unauthorized access. No method of transmission or storage is completely secure, so we cannot guarantee absolute security.
Your choices and rights
You may ask us to tell you what personal information we hold about you, to correct it, or to delete it, subject to legal and contractual limits. Depending on where you live, state privacy laws may give you additional rights, such as the right to opt out of certain uses of your data. To make a request, contact us using the details below. We may need to verify your identity before acting on it.
Children's privacy
Our website and services are intended for healthcare practices and businesses, not for children. We do not knowingly collect personal information from anyone under 18. If you believe a child has given us information, contact us and we will delete it.
Links to other websites
This site may link to websites we do not operate. We are not responsible for their content or privacy practices, and we encourage you to read their policies.
Changes to this policy
We may update this policy from time to time. The effective date at the top of this page shows when it was last revised. Continued use of the website after a change means you accept the updated policy.
